Hi Nancy,
From your list, you seemed to have covered the main items. To make the book more creative:

1. If possible, do the cover in color. A color cover on GLOSSY white card stock paper looks good!
2. Include LOTS of pictures throughout the book. Every couple of pages could contain a collage of family pictures.
3. Include historical pictures in your history write-up.
4. In addition to the history write-up, include a historical timeline of the family history.
4. Do an e-mail directory along with the phone/address directory.
5. Include a family poem written by a family member.
6. Include a family tree or family diagram.
