You can take a peek at my family's form. However, as family members mail in their forms and fees, I am finding that it's not as "user-friendly" as I thought. Also, another factor is that we charge per event, rather than one set cost for the entire reunion. Therefore, our forms have to be more "in-depth", so to speak. Carmen's family form may be more "user-friendly".
