It looks to me like the "Indian Citizen" was just an error on the part of the person that filled out the card. If you browse the "W" surname cards for Tipton County, TN they're all filled out incorrectly just like that. My guess is that the person filling out the card was only looking at the "Citizen/Non-Citizen" boxes and failed to notice that it was in reference to the "Indian" race column. BTW, I don't recall seeing that particular form that they used in Tipton County before but I know that there were many different forms used for WWI Draft cards.
