I had the same problem with my gggrandfather's death certificate. Someone at Afrigenas suggested that I try to locate the funeral home where he resided. It paid off, not only did they have the death certificate but also the names of all the people that helped pay for the funeral. You'll have to make some calls but it's well worth it.
