In all cases, you have to tell the truth as you understand it to be, and these vital records (or online databases) to which you refer, are generally held to be fairly reliable sources of information. If it were me, I'd list all of her mates (married or otherwise) with the appropriate children listed for each relationship.
It sounds like you're afraid that your cousin will be offended by your stating her history as such? Well, that's something that genealogists often have to contend with. Ultimately, the decision will be yours and may effect your relationship with her in the future.
The truth may hurt, but in my humble opinion, it's always the best way.
