One other possibility, is that she may not have died in Pennsylvania (even though she lived there at the time). She could have been visiting another state when she died, so, the death certificate would be in that state. This, even though her funeral was held in Philadelphia. New Jersey is the obvious state to consider. (Many Philadelphians travel to and through NJ frequently)
