The forms vary. It doesn't seem that there was a standard at the time. My great great grandfather's death certificate had pretty minimal information. No listing of his parents. However all of the information was correct from the other information I had. The information it gave me was his date of death, his cause of death, and the interesting information that his body was shipped out of state to be buried.
