Our registration form is on our 2006 Reunion page. We have a standard form that I send to each host family, but they are free to edit and personalize it for their needs. The one on our website now is from the Nassau family.

We don't currently do any sort of assessment or feedback form, though it's certainly a good idea. Let me know how that works for you.
